Form 1004
(Rev3/05) - $275.00
This form is referred to as the URAR form. This is the
most commonly used form for Appraisers and Lending
Institutions because this form contains the most
information. The URAR form is often required for loans
going to the Secondary Market.
Form 1025 (Rev3/05) - $450.00
This is a URAR form for Multi-Family dwellings. This
form is often required for loans going to the Secondary
Market. This is a seven page form.
Form 2055 Exterior Only (Rev3/05) - $200.00
Similar to the URAR, except there is no interior
inspection. This form is limited by the fact there is no
interior inspection.
Completion Report (Rev3/05) - $75.00
This form confirms that work was completed according
to the plans and specifications. Not subject to the
additional “Out of County” fee.
Form 1073 Condominium Form (Rev3/05) - $300.00
This form is referred to as the Condominium Form. This
is used for Appraisers and Lending Institutions for
Condomium projects. This form is often required for loans
going to the Secondary Market.
Form 1004C Manufactured Home Form (Rev3/05) -
$300.00
This is used for Appraisers and Lending Institutions
for Manufactured homes. This form is often required for
loans going to the Secondary Market.
Form 1025 Evaluation for In-House Loans - $275.00
This form is similar to the full report for
Multi-Family with less documentation. It is most commonly
used by lenders for In-House loans. This is a two page
form
Form LND - $100.00
This is a form used when a lender needs to find the
value of vacant land. It is used in the Secondary Market
as well as In-House Loans.
Form 2055 Evaluation for In-House Loans - $200.00
Similar to the URAR report, except the Appraiser does
not calculate reproductive cost. This form has less area
for comparison in the Market Approach than that of the
URAR form.
Recertification of Value - $50.00
A one-page form confirming the value has not changed.
OUT OF BROWN COUNTY FEE - $25.00
